Because every plan is unique, we advise that you contact your insurance company and verify your specific benefit coverage for your desired service.

We are an In-Network Provider for Physical Therapy with Blue Cross Blue Shield.

We are a Non-Participating Provider for Medicare. This means we accept payment at time of service and then submit the claim to Medicare. Medicare and your secondary will reimburse you.

Will Living Well Physical Therapy submit insurance claims for me?

We will submit insurance claims for Blue Cross and Medicare.

What forms of payment do you accept?

We accept payment with cash, check, credit card (Visa, MasterCard, Discover, and Flex Spending Account cards).
