What shall I wear for my treatment?

We suggest you wear loose-fitting, comfortable clothing that allows appropriate movement for your treatment.  For example, fitness attire and/or loose-fitting street clothes are perfectly acceptable.  In some instances your treatment may require a gown for you to wear, which we will provide.

What do I need to bring to my first treatment session?

  • Please arrive 10 minutes early to your first session to allow yourself time complete intake forms. Be prepared to provide your relevant medical history and current medications.  Also, your doctor’s contact information is helpful.
  • To save time, you may download our Patient Registration Form and complete them ahead of time, and bring them with you to your first treatment session.
  • Bring with you a driver’s license and insurance card(s) if you plan to use insurance for payment.  (Click here) for in-network insurance information. Other methods of payment are available (click here).
  • Wear loose-fitting, comfortable clothing that allows you appropriate movement (details here).
  • Bring your calendar for convenience of future scheduling.
  • You may wish to bring X-Ray or MRI reports (if applicable), notes, and a list of questions for your therapist, regarding your condition and treatment.
  • Please let us know in advance if you have received previous therapy treatment and/or in-home health services in this calendar year - this includes physical therapy, occupational therapy, chiropractic, and speech therapy.
